Postfix does not lose mail. If you think otherwise, prove it. You have
logs, if it says something similar to "relay=some.exchange.server
status=sent" in the postfix/smtp log line, it's SOEP - someone else's
problem, not the problem of your Postfix.
If you are using external filters (content filters) or delivery agents
(mailbox_command) or something, make sure these are safe -- procmail is
notorious for filing mail to another folder than you expect, very few
procmail configurations are robust.
