Is there a NAT or load balancer device between your gateway and the
Internet? This device may not be operating correctly. Specifically it may
be losing your session state, which is only relearned if you pre-emptively
send more data.
Make sure that your firewall/NAT/... is not trying to (incorrectly)
interpret pipelined SMTP and is not running out of connection table space.
