Who needs load balancers with SMTP servers? MX records are totally
sufficient.

smtpd processes? These are for incoming mail. Could it be that the LB
switches from one box to another and this keeps the connections to the
Postfix box hanging?

In any case, the smtpd processes should/will die after a timeout period.

I'd suspect the switch. Has the switch been rebooted as well?

trivial-rewrite dies. Which maps are used by trivial-rewrite ?
transport_maps is one of them. Increase verbosity for the
trivial-rewrite daemon in master.cf by adding a " -v"

Postfix is a very good tool to detect network problems.

Consider ext3 for corporate servers.
