Ok, it appears that I had done something wrong (based on the first example
I'd copied perhaps). This configuration appears to be succeeding:

pwcheck_method: auxprop
mech_list: CRAM-MD5 DIGEST-MD5 PLAIN LOGIN
auxprop_plugin: mysql
allowanonymouslogin: no
allowplaintext: yes
mysql_user: USER
mysql_passwd: PASS
mysql_hostnames: DBHOST
mysql_database: DB
mysql_statement: SELECT password FROM mbxtable WHERE login='%u' AND
smtpauth_enabled='Y' AND creation <= NOW()+0 AND (deleteby IS NULL OR
deleteby > NOW())
mysql_verbose: yes

I was strangly unable to find documents describing these options anywhere
except in the distribution itself. :-p
