I have problems with ispell too, may be my suggestions will help.

1)if you are using unicode version, you need to add optional
parameter representing charset to Spell and Affix commands, or this
files must be in charset specified by directive "LocalCharset".

2) Check, did dictionary successfully loaded, when searchd was
started. By default log file of searchd is in var/dlog.log
You must have there smth like "Loaded dictionary ru from
/usr/lib/ispell/ru.hash"

3) Don't forget directive "WordForms on", or "WordForms <lang>"

4) Don't forget to restart searchd after any changes in searchd.conf
